Enrollment Form
Enrollment runs online through Procare. There is a one-time $100 registration fee per family, charged when you submit the form, and it covers every child you enroll.

Most of this goes straight into the form. The records come to our office, and enrollment is not complete until they are in.
Immunization records
Up to date, and on the NC immunization form. We need a copy on file before the first day, not after it.
A completed health assessment
Signed by your child's doctor. State licensing requires it within 30 days of the first day of attendance.
Allergies and medications
Every food and environmental allergy, plus any medication your child takes and the dose. This is what the classroom works from, so it needs to be complete rather than close.
Your pediatrician
The practice name and a phone number, so we are not looking it up for the first time on a day that matters.
Authorized pickups
Your child's birth date, and the people allowed to pick your child up, with phone numbers for each of them.
A card for the $100 registration fee
One time, per family, charged on Procare's site when you submit the form. It is non-refundable, and it is not charged per child.
